Read a finished note
before you sign up for anything.
This is the therapist workspace with sample data in it, laid out screen by screen. Nothing on this page asks for your name, your email or your phone number.
Everything here is invented for this page. No real patient, no real session.
The three things clinicians ask first
Every note is a draft until you review, edit and sign it.
Session audio is deleted automatically after your note is generated.
Your sessions are never used to train external models.

SOAP Note: Individual Therapy Session
Subjective
Client reported increased work-related stress over the past two weeks, describing difficulty falling asleep and recurring worry about performance. Client stated the breathing exercises from the previous session "helped on two of the harder days."
Objective
Client arrived on time and was appropriately dressed. Speech was clear and goal-directed; affect was mildly anxious but congruent. Client engaged actively throughout the 50-minute session.
Assessment
Client continues to make progress toward managing generalized anxiety symptoms. Sleep disruption appears linked to anticipatory worry; coping-skill use is increasing but not yet consistent. No safety concerns reported or observed.
Plan
Continue weekly CBT sessions. Introduce a structured worry-time exercise and assign a nightly wind-down routine as homework. Revisit sleep patterns and coping-skill consistency at the next session.
Sample shown for illustration only, not a real patient or session.
The workspace, screen by screen
A client, a finished note, a homework assignment, a progress view.
Four screens in the order you would meet them. The client, the session and every number are invented for this walkthrough.
A connected client
You share a QR code or a link and the client's app pairs itself to your practice. No account numbers and nothing to type in.

The session, written up
Minutes after the recording ends, the summary, key topics and the SOAP, DAP or BIRP draft are waiting. It stays a draft until you sign it.

Homework, assigned in one pass
Action steps and reflection prompts for your framework, delivered to the client's phone with due dates and reminders.

Progress over the whole history
Mood trend, session frequency, homework completion and recurring themes, so a pattern that is hard to see week to week is visible at once.

The trial runs 30 days and takes no card. Creating the account asks for your work email and a mobile number, because the number is the second factor that protects the clinical record behind your login. We use it to secure your account, never for marketing.
